Guelph Ontario: Epidemic Parotitis in unimmunized children.

ProMED (Harvard) 4 cases reported in unimmunized children. Other unimmunised sent home for 16 days.
Ontario allows unimmunised children to attend schools.

1967 Merck's Dr MAURICE HILLMAN ,1919-2005, produced Mumps vaccine from 5y old infected daughter 's saliva ("Jenny Lynn" strain) Provided free in Ontario but refused by some parents. (Stats`Can 46% Canadians functionally illiterate) Ontario allows children in Secondary education .to work;usually in fast-food chains.

COUNCIL MEETING Ontario College Physicians & Surgeons.(CPSO)

In response to Liberal political pressure, details of all complaints and resultant action to be published on-line.. Also the CPSO will list all non-Ontario licences`of members  that it  knows about. (The CPSO notifies all other known Licencing bodies about result of complaints.)

About 1000 Ont.MDs act as agents of the CPSO assessing a practice every 10 years; after 70y every 5y.
These doctors are mainly not Academic and practice Ontario Guideline medicine. A danger for MDs who are Internationally up-to-date.

In Ontario, less risk to career  to practice  Academic medicine, preferably non-Clinical,..

MEMO: CPSO used to delete CPSO DISCIPLINE TRIBUNAL (not a Court) if verdict reversed on Ontario Court Appeal.. CPSO will now NOT delete Tribunal verdict even if MD wins on COURT Appeal.. Difference between a TRIBUNAL & COURT not generally known. One point: in TRIBUNAL no need to Swear or Affirm. Also no need to stand.when Tribunal enters and leaves.

Left-wing medical activists worried about rise of private doctors' offices and private hospitals e.g. Montreal Rockland MD.and Vancouver Cambie Surgical Centre.. In Toronto: "Wellness" GP clinics ; Cleveland Clinic Canada;  MedCan;  MediSys (Montreal,Toronto,Calgary,Vancouver).  Usual fees are $3,500 a year. GP visits NOT billed to OHIP.;.

Since 1947 UK has had private and public medicine. Population increasingly insured privately e.g. with BUPA Many private hospitals including private mental hospitals..

Specialists who wish to work for NHS sign a 9/11 sessions a week contract. 9 x 3.5 hr sessions. Leaving two half-days for private practice at market price. Get pension at 65y.when compulsory retirement.from NHS. Can then practice 100% privately according to market demand.

Some UK specialists.work entirely outside NHS.. Can perform surgery in private hospitals.

Unlike Ontario, talent is rewarded by A,B,C, Distinction awards. A= Doubles NHS rate for FULL-TIME
academics with NO PRIVATE PRACTICE. B & C depend on Research & Teaching. In Ontario all Specialists are paid the same. by OHIP.. In Ontario Medical Specialists supplement their income through Drug Trials.Some surgeons commute to USA & UK.Can also bill market rates to non-Canadians..

Naive to believe that Ontarians paying millions for a condo or home; who pay upto $50,000/yr for private schools (not tax deductible) and are members of expensive private sport and social clubs will  accept  OHIP standard of medical care.. Prefer luxury of private clinics which are disguised as "WELLNESS CLINICS" & offer extra services such as Dietitian +/- Physiotherapist..Cardiologists have combined to open elegant offices with advanced imaging.billing OHIP and accepting private out-of-Province clients.

OMA COUNCIL MEETING.(May 1-3)

  1. Approx 250 elected Council members are paid to attend. Based on Districts where they practice and Branch of medicine.  Cost of three-day meeting at  Hilton Toronto 145 Richmond St including meals is about $1-million. NON-DELEGATE OMA members can attend as observers. Do not vote. May be allowed to speak.

Districts to be reduced from 142 to 125.  Some were a`legal fiction. e.g. In Toronto,Central, Doctors & Wellesley Hosps were closed  many years ago.  Sections reduced from 63 to 49. In past a section could be formed.by 25 members.(now 50). Section delegates increased from 96 to 102. New Section of Academic doctors.

New Medical Interest Groups (50+) without a vote and not paid to attend Council can be formed. The OMA will provide about $5,000 for say 2 meetings a year of a` MIG Executive:: Chair + Sec/Treas+ Past Pres +  2 Directors who are paid to attend.. Food catered.

MIGs now accepted: SLEEP STUDIES. & SURGICAL ASSISTANTS.
MIGs pending: Complementary medicine and Clinical hypnosis.

New OMA Pres. AYLMER Ont (S/E of London) popn.7,100+/-  GP  Michael Louis TOTH MD (U West.Ont1984)..

Much of Council meeting spent on MOH pay cuts and leaving Ont.MDs without a contract. NPs, PAs and Pharms. taking over roles of GP. NPs can now prescribe non-narcotics and refer pts to Specialists. Pharms can alter GP prescription.without permission.

OHIP pays same throughout Ontario. Obviously rental overhead varies. OHIP cuts encourage move out of high property value areas. In Aylmer 4-bedroom home on one acre about $400,000. In Toronto would cost several million.

GPs moving into NON-OHIP paid medicine:  COSMETIC MEDICINE.and opening WELLNESS clinics which provide extras e.gl. Dietary advice +/- Physio. OHIP is NOT BILLED for GP services. Patient pays about $3,500 yearly. This legal loophole is tolerated although supplementary billing to OHIP fees is not allowed.. Many Ontarians subscribe to Cleveland Clinic Canada,  MedCan and MediSys..Health Group.

There is lack of unity in the profession. Sexologist, U.Tor Lecturer Frank SOMMERS MD (Tor.1970) FRCPC (1975) accused GPs seeing 60 patients a day" of cheating the system. It had to be explained to "social activist" Dr.Sommers that GPs often work a10 hour day. So a patient every 10 minutes is reasonable.

To encourage new ideas, there is now a 20-year limit to OMA committee work. As hourly rate increases`with number of committees some MDs have made the OMA a part-time job collecting committees..Especially as the OMA pays for travel across the Province and 5-star hotels.
